All players are required to fill out Registration and Medical Release Forms each year.

Volunteer Form

Anyone volunteering in B.B.O. that will be in contact with players at the fields, including games, practices, concession stand, field maintenance must fill out and submit the volunteer application!  This must also be done each year.

12 Year Old Waiver Form

Parents may use this form to request that their 12 year old player not be put on a Major level team.  Must be approved by the Board of Directors and District Administrator.

Tournament Forms

Tournament Application form is required for all players wishing to participate in post season district tournaments.  Final team rosters will be decided either by a vote of Managers, Coaches and the Board, or by tryout, depending on level.
