All players are required to fill out Registration and Medical Release Forms each year.

Volunteer Form

Anyone volunteering in B.B.O. that will be in contact with players at the fields, including games, practices, concession stand, field maintenance must fill out and submit the volunteer application!  This must also be done each year.

12 Year Old Waiver Form

There are new rules this year which require that all 12 year olds be placed on a Major team.  Parents that do not want their child to be automatically placed on a Majors Team may fill out and submit this form for a waiver.  PLEASE NOTE:  This must be approved by the board of directors as well as the District Administrator, and is not guaranteed.

Tournament Forms

Tournament Application form is required for all players wishing to participate in post season district tournaments.  Final team rosters will be decided either by a vote of Managers, Coaches and the Board, or by tryout, depending on level.
